FMC's VBS is for children who are kindergarten eligible through completion of 5th grade.

A Parental Consent form must also be completed for each child.
Consent forms are available to download on this web page.
You may send the consent form with your child, mail it to us, or
email a photo of it.
Please specify if you will allow your child to be photographed
and/or in a short video of activities.
